FOP2 justs starts chan_spy with the "w" option. I heard some other reports like yours. I believe there is a buggy Asterisk version outthere... but not sure where. Asterisk based distros usually use patched asterisks, and they can have bugs like that..
I have just found out that the latest trixbox does not display UserEvents in the manager unless you add the "hud" permission to the manager user. Those UserEvents are needed by some FOP2 non essential features (like presence display).
