Action buttons missing

I have a server that is setup with FreePBX and one user is missing the action bottons at the top of his screen. I have the promissions set to all on his account in FreePBX but none of the buttons show up at the top including the settings and logoff button. This user was working fine before but after the weekend everything broke. All other users are fine and working. I can email a debug output if needed.

    Is the extension listed as a button? If not, action buttons won't appear. Your extension must show if you want the action buttons to appear.
