Change Password extension not working

I am able to change my password in the Admin control area, but when I log in as a user I cannot change my own password. Change Password v1.0.1 Plugin is enabled. Other users seem to have the same issue when I log in as them.

I always get "Bad Credentials" after hitting Submit, even though I've verified they are correct.

Version: 2.31.29 on FreePBX

    Other users in our system have this issue, and I've tried a different browser, Edge, and still always get "Bad Credentials" when attempting to Change Password within FOP2 UI.
