Calls Randomly Get "Stuck" On A FOP2 Extension

Hello!
We are experiencing a recurring issue with FOP2 Manager 1.2.3 on FreePBX 13.0.197.28 where calls will randomly get stuck on an extension and do not end after the caller hangs up. This call will then continue to show active until the FOP2 service is manually restarted and the user is treated as "on a call" for queuing purposes as well.

Is this a known issue, and if so, are there any ways to work around this problem? Here is an example:
