I have a few questions about transferring calls:

(We have drag-transfer enabled, in case it's relevant.)

Can transfer to queue be enabled somehow?
When I'm on a call and I click a queue, then click either transfer button, I hear a few button press tones, but the call stays on my end. It does not attempt to transfer.
When I attempt to drag-transfer, nothing happens.

Can transfer of a held or unanswered call somehow be enabled?
Our operator will sometimes field several calls at one time. While she's on, she will often times see a caller who she knows needs to go to a certain phone or queue, but she has to answer the call in order to transfer. This causes the wait and hold times to go up.
I'm unsure if it's possible, as attempting to do so either via the transfer buttons or drag-transfer does nothing.

If you hear dtmf when you attempt and attendant transfer it means you do not have the tT options in your original dial command. Add that and supervised transfers should work. (Blind transfers should work regardless of dial options tT)

Supervised and blind transfers normally work, just not when there's a call on hold.

Transfer works only on active calls, not held ones. I have to modify dragtransfer to enable transfer of held calls too.

Part of the issue is that the active call won't transfer, either, as long as there's a held call for that exten.
