FOP2 addon installation error

Kindly assist, I got the following error messages during addon installation of fop2 on elastix 2.2

[root@asterisks ~]# /usr/local/fop2/fop2_server --test
Flash Operator Panel 2 - No valid license found. Demo Mode
ERROR 1146 (42S02) at line 1: Table 'asterisk.fop2settings' doesn't exist
ERROR 1054 (42S22) at line 1: Unknown column 'exclude' in 'where clause'
Manager authentication failed. Check credentials!

You must install the latest fop2admin version (1.2.8).

Thanks, this worked like magic and all error messages gone, but I still have an issue, the action buttons are not working. I also see this error in my log manager.c: 127.0.0.1 failed to authenticate as 'admin'

check that your manager credentials are set correctly in fop2.cfg as defined in /etc/asterisk/manager.conf.

Thanks, but below are my settings, the action buttons still do not work and I still have this error:
