I read that few other users reported similar issue but there was no solution.
we are using FOP 2.31.30 on freepbx15 with asterisk 16.19
we are using *11 and *12 for hot desking and agent login/logout on IP-Phone. we frequently get issue that FOP2 shows the extension left busy and we can only clear by restarting asterisk and fop2
Kindly advise what other information do you need to look at this matter.

Hi everyone,
we have a scenario where customer having two freepbx server and using advance recovery module from sangoma to setup sort of high availability.
the advance recovery module is not really HA and each pbx has their own IP etc and at each pint of time one of them would be active.
would like to check if we can use fop2 on external server and link to both servers, how would be queue agent usage? we use fop2 for agents to login to queue and monitor queues.

clearing cache does not help
this server had major problem and we di fresh installation of freepbx 13 and restored the old database, fop2 also installed again and we found new installation is different version from old one, there might be issue with old data restored and conflict with new fop2 version

we have just installed new version (2.31.22) and facing problem with the calls originated from phone book, seems the call origination structure has a problem, see bellow debug:
